Examples of some qualification questions:
• Where were you at the time of the incident? This may be relevant in relation to other workers, objects, etc.
• What were you doing at that time? Duties, tasks, explain if necessary.
• What did you see, hear, feel (changes in light, vibrations, heat, cold, wind, etc.)?
• What were the environmental conditions (weather, light, noise, etc.) at that time?
• What were the injured/involved workers doing at that time?
• What do you think was the cause of the incident?
• How could similar incidents be prevented in the future? What would you (witnesses or employees involved) do differently?
