A /"perfect dodge/" is performed by tapping the left stick left/right at the last second before an enemy hits you. If done correctly, the dodge will be in slow motion. DO NOT HOLD THE left stick! You only need to TAP it briefly when the enemy is about to strike. The timing is tricky to get right, but on PS5 the trophy has a built-in trophy tracker and counts via Checkpoint Restart, so you can farm these one at a time, or just try it from one enemy to the next as you play. A good place to do this early on is the 2nd enemy you encounter in the game, in Chapter 2: Outbreak (after the 1st enemy where you get the combat tutorial).
