New: A defect is identified and reported by a tester or other stakeholder. It is usually logged in a defect tracking system or problem management tool. At this stage, the defect is considered "new" and awaits further analysis.

Assigned: The defect is reviewed by a team member, such as a lead or manager, who assigns it to the appropriate developer or tester who is responsible for fixing or reviewing the defect.

Open: The defect is being actively worked on by the assigned developer or tester. They investigate the defect, reproduce it if necessary, and analyze its root cause.

In Progress: The assigned developer or tester is actively working on resolving or fixing the defect. They may make code changes, run tests, or make any necessary adjustments to fix the issue.

Fixed: The developer or tester has implemented the necessary changes or fixes to fix the defect. The defect is considered "fixed" and ready for further review.

Pending retest: After the defect is fixed, it is returned to the retesting tester. The tester verifies whether the fix successfully resolved the problem or whether new problems were discovered during the fix.

Retest: The assigned tester performs a test on the fixed defect to ensure that it has been fixed and that no regression or new issues have arisen. If the defect passes the retest, it moves to the next phase.

Verified: The tester confirms that the defect has been successfully fixed and retested. It is marked as "verified" to indicate that the issue has been fixed and is ready for closure.

Closed: The defect is officially closed, meaning it has been fixed, retested, and verified. It is no longer an active issue in the defect tracking system.

Reopened: In some cases, a previously closed defect may re-occur due to a recurrence of the issue or incomplete resolution. In this case, the defect is reopened and goes back into the defect lifecycle, starting at the Open or Assigned phase.

The defect lifecycle helps teams track and manage defects effectively and ensures that issues are properly addressed and resolved before software is released to end users. It provides clear stages for defect progression and enables stakeholders to monitor the status of each defect and take appropriate actions to maintain software quality.
