Twenty percent of obese people have a very healthy metabolism. Meanwhile, sixty percent of people who are supposedly of a "healthy" weight suffer from poor metabolism and are prone to a range of chronic diseases. With this in mind, do we need to change the way we think about what "being fat" really means?

Today, I'm delighted to welcome back Dr. Robert Lustig. He is a leading authority on public health and has spent many years trying to expose the truth behind the food industry and the many myths of modern medicine. Rob is Professor Emeritus of Pediatrics in the Division of Endocrinology at the University of California, San Francisco. He is also the author of several books, including Metabolical: The Truth About Processed Foods and How They're Poisoning People and the Planet, which was published in 2021.

Robert first joined my podcast in episode 251, when we took a deep dive into what happens in our bodies when we consume too much sugar. In our conversation today, we'll revisit the impact of sugar on human health, but the focus will be on the three different places in the body where we store fat: the main types of fat deposits: subcutaneous (visible and palpable); visceral (stress-related fat in the middle of the body); and liver fat. Only the first of these fat deposits is likely to be noticed on the scale—but the latter two, Rob says, really determine your health.

We talk about why it's stress, not diet, that drives the formation of dangerous organs – the fat that surrounds our organs – and Rob's view that chronic stress strengthens metabolic, mental, global and planetary health.

We also discuss
– how a diet with as little processed whole foods as possible can reduce both stress and weight gain
– why he is not a fan of fructose and the glycemic index
– why he believes continuous glucose monitors (CGM) can help the general population, not just diabetics
– why the liver might be the body's most important organ when it comes to metabolic health and some of the most important tests we can perform to assess the current state of one's liver health
